VISA FOR VISITING RELATIVES REQUIREMENTS TO BE PREPARED IN THE PHILIPPINES

申請者が比国で準備するもの

1.Fill out visa application form
　査証（ビザ）申請書（在マニラ日本国総領事館備え付けのもの）

2.Submit two (2) photos (4.5 x 4.5 cm) - photos must be pasted onto application form
　写真２葉（サイズ、4.5×4.5㌢）

3.Copy of philippine passport - including all pages with visa and immigration stamps
　比国パスポートの写（氏名、写真、出入国のあるページ）

4.Certified true copy of birth certificate issued by the national statistics office (nso) (security paper with nso logo at the back - not retyped) in case of no record of birth at nso, birth certificate by city hall and authenticated by nso as well as certificate of non-record of birth by nso
　出生証明書（National Stastics Office「NSO」発行のSecurity Paperを使用した謄本）

　　（注）NSOに記録がない場合には、市町村発行の謄本でNSOの認証印があるもの及びNSO発行の出生記録在証明書

5.Certified true copy of birth certificate of filipino relative(s) in japan (same as no. 4)
　日本在住の比人親族の出生証明書（同上）

6.Documents to certify applicants occupation/profession - e.g. certificate of employment, school certificate, copy of business registration
　職業を証明する書類　（在職証明書、会社登記証、在学証明書等）

7.Documents to support financial means for travel expenses - e.g. copy of income tax return (itr), bank certificate(s)
　渡航費用、滞在費の自己支弁能力を証明する書類（納税証明書、預金残高証明書）
《申請者が被扶養者の場合は扶養者の書類》

8.Self-addressed/stamped return envelope - result of your application for visa will be forwarded to the name and address indicated on the envelope
返信用封筒（申請者の氏名・住所を明記、切手添付）

REQUIREMENTS TO BE PREPARED IN JAPAN
日本に住む身元保証人が準備するもの

1.Certified true copy of family register (Koseki-tohon)
　戸籍謄本

2.Residence certificate
　住民票

3.Certificate of employment
　在職証明書

4.Certificate of income tax payment<
　納税証明書又は源泉徴収書

5.Detailed statement giving reason/s for invitation
　招聘理由書（具体的に、日付及び署名は自筆とする）

6.Copy of passport - include record of entry into the philippines
　保証人の旅券（パスポート）のコピー（氏名、写真、比国の出入国スタンプのあるページ）

7.Copy of passport of filipino relative(s) in japan (first two pages only) and copy of alien registration (both sides)
　日本に居住する比人親族の比パスポートのコピー（氏名、写真のあるページ）及び外国人登録証明書（裏表の両面の拡大コピー）

NOTE: ABOVE-MENTIONED DOCUMENTS SHOULD BE VALID FOR PERIOD OF SIX(6) MONTHS
　　　　以上１.及び２.の書類は作成日より６ヶ月以内のものに限る。

NOTICE: In Principle, some documents submitted to our office will not be returned. Important documents such as ID Card, Professional License, Business Registration, Business Permit, School Diploma, etc., which are difficult to obtain again, must be submitted in clear copies, but not in the original. ITR, if requested, must be submitted in original copy, but shall be returned after a decision on your application has been made. In case any of the original documents mentioned above was mistakenly submitted to our office, please report to the staff at window together with the following information:
　提出された書類は返却しませんので、ご了承ください。ただし、免許証、会社登記証、営業許可証、身分証明書、卒業証書等、再発行してもらうのが困難な書類については鮮明なコピーを提出して下さい。誤ってこれらの書類の原本を提出し返還を希望するた場合、窓口の担当者にレシート番号、ビザ番号と発給日（又は拒否番号と拒否日）等と併せて申し出てください。

1.Receipt Number;
2.Visa number and date of issuance;
3.Denial number and date of denial.

PROCEDURE FOR APPLICATION
　申請方法
1.Submit filled out original visa application form - all other documents should be copies (one of each) keep all original documents until requested
　上記の提出書類は、査証申請書、写真及び返信封筒については原本を、その他の書類については全　て写し（コピー）を各１部、提出してください。なお、原本（オリジナル）は別途提出していただ　きますのでそれ　まで保管して下さい。

2.Put above-mentioned requirements into an unsealed brown envelope (do not fold or staple documents) insert into drop box located at entrance of visa section
　申請は書類を大きな茶封筒に入れ、封をせず提出して下さい。

3.Do not submit original philippine passport
　申請者の比国パスポートは絶対に同封しないで下さい。

4.In addition to the above requirements, additional documents may have to be presented upon request
　上記の他、追加書類を求める場合もあります。

5.Documents may be submitted on weekdays (except wednesdays) from 11:00 a.m. to 12:00 noon and 2:30 p.m. to 4:00 p.m.
　申請時間：午前１１時～１２時、午後２時３０分～４時　（水曜日、土日、祝祭日を除く平日）（在マニラ日本国総領事館休館日）
6.Telephone inquiries may be made weekdays in english and tagalog from 3:00 p.m. to 4:00 p.m
　電話による質問・照会時間　：英語・タガログ語　午後３時～４時　　　　　　　　　　　　　　　日本語午前11時30分～12時30分、午後４時30分～５時

MISCELLANEOUS その他
1.Results of application will be mailed and may take at least two (2) weeks to arrive - it is advisable to submit application form and necessary documents one (1) month ahead of desired departure date
審査の結果は返信用封筒を用い、郵便にて回答しますので、早くても２週間以上を要します。従って、査証申請は出発の１ヶ月以上前に、十分な余裕をもって行って下さい。

2.Applicants who submit wrong information or falsified documents will be immediately disqualified
申請書に虚偽の記入や偽造書類を提出した者、または過去にそのような申請をした者は、査証発給の対象になりません

3.Submission of requirements does not guarantee issuance of visa
提出書類が整っていれば、必ず査証が発給されるというものではありません。

4.Reason(s) for declined applications will not be disclosed in any form of inquiry
査証発給拒否の理由については、問い合わせがあっても回答できません。
